After defeating Tiamat, return to Samael and after a cut scene this ability/achievement will be yours

This achievement is story related and cannot be missed.

Once you've defeated Tiamat return to Samael. Upon giving him the first of the 4 Chosen hearts a cutscene will ensue. Once finishing you will be reborn with the Chaos Form ability.

You will also earn 20G for your efforts!

Part of the regular story progression. No special work necessary. Impossible to miss. Below is a detailed spoiler of the event that unlocks this trophy.

After handing Tiamat's heart to Samael there will be a very interesting cutscene. In it War regains his ability to transform into "Chaos", the red demon form which you originally had at the start of the Apocalypse. After the cutscene you will unlock the trophy.
