In chapter 4 when in the back ally with the book store and pharmacy you will need to travel to a blown out building located up some stairs to grab the warehouse key. On the ground in the building is what looks like bits of gore on the ground, interact with the ground to unlock the achievement. Below is a video with the location at 1:30.
