You can start working on this during chapter 2 and only while playing as Sam Walker. One of Sam Walker's quirks is that he has hallucinations every now and again during normal game play. Most of the time the screen will tinge red to indicated a hallucination is happening, if there is a zombie present shoot it with any gun (preferably the handgun) they will dissolve away into a red/black effect.
Do this 5 times during a playthrough of the game and you will unlock the achievement.
