How to unlock the Disarm and Dismember achievement in Dead by Daylight - Definitive Guide
Special items are anything that can be created or found in a map but not taken out of the match; (not medkits, toolboxes etc.). There are a few killers that give the survivor the option of using the special item. Currently, the only way survivors can create a special item is Leon's Flashbang perk.
For this achievement, you must hit/attack a survivor holding a special item. A survivor can be hit multiple times during a match.
Best ways to get this:
1. Wesker or Singularity. Look for a survivor holding the spray or EMP device respectively. Down them, let them wiggle free, chase them and down them again. You are probably more likely to be able to grind this quick with Wesker instead of Singularity due to the EMP having the option of getting used while survivors are running.
2. A more passive way to get this by hitting a survivor holding a flashbang. This method will not be as quick but if you have many other achievements, this one will likely happen on it's own.
Special Item:
Nemesis: Syringes are consumed after survivors use them so not likely to get a hit before the item is gone.
Cenobite/Pinhead: Teleport to the survivor that is try to solve the puzzle. GOOD
Onryo/Ring: Survivors typically don't deal with the tapes unless they have to. So, rare to hit them while holding a tape. -------CURRENTLY BUGGED/NOT WORKING
Wesker: Look for the spray bottle in a survivor's hands.
Singularity: Look for the EMP in the survivor's hands. If they use it, it will disappear.
Xenomorph: It is pretty unlikely that a survivor will carry these very far and allow you to hit them with one.
Using Wesker, I was able to grind all 50 hits in 3 matches. I did have to bully a few survivors the entire match in order to get this done.

am I the only one getting 0 % progress even if I hit someone with the listed things?

VHS Tapes currently do not count for this achievement.

My solution will all but guarantee you get this achievement IF and ONLY IF you don't care about getting sacrifices and winning.
The killer: Pinhead
The addons: Chatterer's Tooth (Purple, reveals location of box to you and does not cause chain hunts if youre the one to pick it up) and Torture Pillar (Green, makes chain hunts activate 6 seconds sooner)
The perks:
1. Insidious: standing still makes you lose your terror radius until you move.
2. Coup De Grâce : each token, earned by gens being done, extends lunge attack by 80%
3. Save the Best for Last: 40% faster weapon wipe speed at max tokens.
4. Either Brutal Strength: 20% faster pallet breaking or Spirit Fury: breaking 2 pallets causes the next one to stun you to instantly break.
The map: Doesn't matter to be honest, but Midwich was perfect.
The method:
This is going to sound incredibly boring (because honestly it is) but what youre going to do is go to the box, find a hiding spot, stand still and just wait for someone to come get it. Once they do, charge at them and hit them.
The faster activation of chain hunts usually helps to spur survivors into getting the box, and because the imterruption caused by chains during the hunt is so annoying, even if they catch on to what you're doing, 9 times out of 10 they're gonna come get the box. Within less than 5 matches I had 34 of 50 hits. It works beautifully but again, only if you don't care about winning.

Despite Hoarder no longer working on special items as of 7.0.0, I still think Pinhead is a good choice. You still see their location when they start solving it, and you can use add-ons to increase solve time and casting range to consistently interrupt the box, which is a generally effective way to play him. You’ll naturally get it as Wesker, but Pinhead will get it done with better results.
If you’re going to do it as Wesker, try to run the iridescent add-on that starts them off infected, and then only hit them with basic attacks the whole match. They’re likely to hold onto the first aid sprays after being forced to pick them up at the start. If you don’t have any of those add-ons, just work on infecting them each once on your own, and then use only basic attacks going forward. The fewer sprays they’re forced to consume, the better.
Also, use survivors tea bagging at the exit gates to your advantage - they might be holding onto special items when you hit them out.
If you’re going to do it as Wesker, try to run the iridescent add-on that starts them off infected, and then only hit them with basic attacks the whole match. They’re likely to hold onto the first aid sprays after being forced to pick them up at the start. Singularity is also a good idea since survivors often carry an EMP until they find a use for it.
