How to unlock the Item of Obsession achievement in Dead by Daylight - Definitive Guide
In order to attempt this achievement you must have the perk "OBJECT OF OBSESSION" unlocked.
This is a Laurie Strode unique perk. I don't know if you're required to have purchased The HALLOWEEN Chapter or if the teachable version perk can still show up in the shrine but obviously buying the DLC is the fastest and most sure fire way to get it. Simply level up Laurie to around level 10 and it should have showed up in your bloodweb at some point.
Now with the perk equipped and once you get in a match you should see little bones around your character icon in the lower-left of the screen. If you don't see it then that means someone else is the killers obsession and you cant get this during that match as the obsession will never transfer between players.
What the obsession does is that the player that has it and the killer will be able to see each other through walls within a certain radius away from each other. The lower the level the perk, the bigger the radius.
You might want to level your character up a little more and unlock more perk slots to help you avoid the killer better and be able to contribute to make the match go faster.
Two very important things to remember. You can not get hit. If you get hit, you can not get the achievement that match and healing won't matter. You must be the LAST person in the game to escape. The other players can escape, die or even disconnect but you and the killer need to be the last people in the match actively playing.

You don't need the perk, if the killer is running a perk that makes you the obsession you can still obtain it. Got it tonight on David King when killer ran a perk that made me his obsession.

Obviously that's more of random chance but I'll include that info into the guide.

You can cheese this if you have friends:
Be in a four man swf, ensure no one else has any perks that increase your odds of being the obsession.
Equip the perk "sole survivor". Optionally, there are others that will help you. (I used left behind, spine chill and lightweight)
Equip offerings amongst your group to ensure the hatch spawns in the killer shack, and a map offering that has a shack.
Once you're in the game, get to the shack, get your friends to all DC (pause - leave game) and the hatch will spawn right next to you. Might take some rng as the killer may spawn nearby, and some killers may affect it, but should only take a few tries.

If you want to go for this achievement, it's recommended you use a perk that increases your chances of you being the obsession. All of Laurie Strode's perks do this.
During the trial, use a stealthy playstyle. Lightweight or Distortion might decrease the chance of being tracked or detected.

This is challenging to pull off. As a survivor you need to be the obsession, which means either you need to randomly be targeted by a killer's obsession perk at the start of the match, or be playing obsession perks of your own. There can only be one obsession per match. If you are the only person playing an obsession perk you will be the obsession. If multiple survivors are using an obsession perk, the obsession will be chosen at random between them. If you're using multiple obsession perks you will have a greater chance of being the obsession than anyone else also using them. The obsession will be identified by the claws surrounding their health icon on the HUD. The last pitfall is that when you're the obsession, no other survivor can trigger the Decisive Strike perk which will then make that player the obsession even if there already was an obsession in the match. You can earn this trophy playing as any survivor, and even if you don't own the DLC. The Trophy will pop on the results screen post-match if the conditions were met.
This trophy was intended to get you to use the Laurie Strode perks "Object Of Obsession" and possibly also "Sole Survivor" to evade the killer the entire match and then escape untouched. I found it to actually be a hindrance though since the killer can see you too. Today there are much stronger options for staying off the grid such as:
Distortion (Jeff perk)
Urban Evasion (Nea perk)
Premonition (Basic perk)
Spine Chill (Basic perk)
Calm Spirit (Jake perk)
Alert (Feng perk)

you need to escape from the map where Myers’ house is (there are police cars there). At the same time, you should NOT BE HITTED ONCE.

Moroz473 was a little misleading. There are only 3 mandatory conditions - to be the obsession of a maniac (the easiest way to become an obsession is to put any Laurie Strode perk), to escape last and not receive a single blow from the maniac, the maniac and the card are not important

Escape through the hatch does not count on PS 5
