Playing on a friends account: still bugged as of 05/04/21 but something that’s a little bit of leverage is that as long as you leave the trial when only one Survivor has gone down. It still counts.
If you’re having a game where you’ve whitewashed the killer. Sit by the exit and get downed, crawl out as the original method used to be.
Alternatively. If the one and only Survivor has been downed then hooked for the first time and the gate is open. Just leave. It’ll still count

Just a little more clarifying and a hint, you must get to the dying state only once in a trial, does not matter if you get hooked or not.
More often than not, you will find yourself in a match where you were not harmed, and got through to the exit gate. Use these occasions to wait for the killer right in front of the exit, so that he hits you one or two times, only enough for you to make a crawling escape, which will count for the achievement.
By the way, the achievement tracking in the xbox app/dashboard actually works for this one.

I dont know what i am doing wrong, but i cant get progress for this achievement. I have been playing a lot and my tracker only stays at 32%. Even though i know i met the requirements it still doesnt go up. I think i already escaped 50 times while going down 1 time and being hooked. But im still at 32%

I am seeing the same issue as Synizzer. The bar does not go up even when I go down, get healed and escape without being hooked.

This can be done on any map and does not require any DLC to be purchased.
Basically get knocked down to the ground so that you're crawling once and then escape via an Exit Gate. (Escape via a hatch does not count.)
Ideally what you'll want to do during the match is avoid getting downed at all as if you get downed a second time then you'll forfeit any progress towards this for that match.
Once you get the five generators and the gate open, what you want to do is position yourself an inch within the invisible wall that you have to pass through to exit the game then wait for the killer to down you and hopefully when you fall you'll fall through the invisible wall and be safe. This can be hard to get right and you may screw up a couple times thus giving the killer the chance to pick you up and hook you but you should have no problem getting it most of the time.
If you ever do get downed in the match, play super defensively for the rest of the game.
Perks I recommend:
-Premonition
Probably the best perk period. Lets you know the killer is coming from the direction your camera is facing. (This is especially good right now that everyone is playing The Pig as their crouch speed is so slow that you'll be able to detect and get completely out of an area before they even show up.)
-Lightweight
This will hide your trail quicker and help you juke killers better.
-Calm Spirit (Jake perk)
You won't alert crows. Helps when trying to juke or generally to stay undetected.
-Self-Care (Claudette perk)
You can heal yourself without a medkit so you'll avoid going down.
David King and Meg have helpful perks but I feel the above ones are much better. Laurie has the Decisive Strike perk which will let you escape the killers grab one time but it's more likely that they'll just down you a second time than you get away, so you're better off with just getting hooked and a teammate maybe saving you.
Figure out what works best for your playstyle.
One thing to remember is that you can get hit once to be put in to the wounded/limping state and then heal yourself as many times as you want.
I believe if a killer grabs you while vaulting through a window or over an object will count as a down but I was not able to confirm that.
After I escaped I always opened up the guide menu and went to the achievements to see that I got the 4% progress to the achievement tracker. The escape should count as long as it gets right to the end game score screen. One time my game crashed right as I got to this screen but the progress showed up a second before the crash. If your game crashes at any time before then you probably won't get progress tracked for that match.
If you're solely going for this achievement and you get downed a second time, you can always pause and just quit out of the match. You'll lose rank pips and any bloodpoints you would have earned that match but you'll save time.

It's worth noting that this achievement is currently very buggy. I had 18 games that met the criteria that didn't count. So if anyone thinks they're doing something wrong because they didn't get progress, you're probably OK. You'll just have to do it again.

This can be earned by any survivor. You will need to survive to the end of a match and escape while only ever being put into the dying state once in the match. You do still have to be put in the dying state so matches where you weren't downed at all don't count. This trophy will pop on the results screen post-match after your 25th single down escape. This can be unlocked even if you don't own the DLC.
My advice for this is to play very cautious and survive to the end without getting downed at all. After the gates are powered you can stand close to the edge of the exit and let the killer hit you. Just take special care that you're close enough to the edge that when you fall down you fall out of the play area and escape so you can't be picked up and hooked.
