Each Vecna trial has special chests that spawn around the map. When survivors open these chests a dice will roll and in the event that the survivor rolls the number 20 they'll receive the eye or hand of Vecna.
In order to unlock this achievement you must get a survivor who posseses the eye or hand to death hook (hook them twice) and once you've downed them for the final time you'll be given a prompt to perform a special mori on them. Now you must kill every other survivor (4 kills total) to unlock this achievement.

you can just mori the Eye/Hand Holder and sacrifice the others it will still count

How do you know if someone has the Hand/Eye? Could you add this to the guide?

To retrieve the eye or hand of Vecna, you must wait until a survivor collects one of them (indicated by a voice line along the lines of "that isn't yours" "you don't know what you took" etc... and a little icon next to their survivor profile on the bottom left of the screen).
Then you must get that survivor to death hook (the same way you would if you were looking to Mori them) and you should have an option upon downing them to retrieve your body part. This will act as a very quick mori.
From here all you need to do is kill the rest of the team!
