The Doctor is a very easy and very fun Killer to play as.

This achievement simply requires you only to hit 500 Survivors with Shock Therapy.
It takes time and will take many games but you'll come to love playing as the Doctor and you'll rack up a considerable amount of Bloodpoints during your play.

Shock Therapy is the Doctor's ability. You shoot a small a amount of electricity a few meters in front of you to hit a circular area of effect on the ground.
Any Survivor within the area is effected by Madness.

Madness is a whole heap of fun and will be the bane of most Survivor's existence. Madness comes in Tiers 1-3. 3 being the highest and makes a Survivor pretty much functionless. Whilst 1 is just a minor inconvenience.
You can see the stages of Madness in the bottom left screen at each Survivors icon.

Now my advice is to get each Survivor to the Tier 3 Madness. Then start to down them. As a new Killer. You'll ten easily locate some of these Survivors once they've been rescued.
This method will give you a great balance between experience, win/lose ratio, Bloodpoints, Map knowledge and Survivor hiding spots.

Herman's had a few reworks and I believe this achievement was unobtainable for a short time. But seems to be obtainable again!
