How to unlock the Terminal Illness achievement in Dead by Daylight - Definitive Guide
While playing as The Plague "Doesn't matter the level of her." Her special ability "Vile Purge" allows you to "puke" on players and objects that they can interact with. The Plague has two different types of vomit: her regular vile purge (slime green), which infects survivors, and then her corrupt purge (blood red), which hurts people. When you hit someone with the regular green vomit, they get infected. At this point, if you and they do nothing, it will take 90 seconds for them to get sick (maximum infection), which means they are one hit and cannot be healed regularly. This process can be sped up if they do things like sprint, do generators, heal others etc, or if you keep puking on them.
You will get your red vomit/corrupt purge, when someone who is sick (maximum infection) cleanses at one of those fountains, which will turn it red and when you then drink from it, for 40s (can vary if you have certain add-ons).
For this achievement, you have to down people, be it by hits or red pukes, who are infected/broken/one-hit. A good way to do this efficiently is get everyone infected quickly by using a perk like corrupt intervention and infecting all generators, which will then infect anyone working on them, or use lethal pursuer and find people quickly that way. As soon as they are infected, leave them and turn to someone else. If your opponents refuse to cleanse to deny you your red puke, they will all end up one hit. Many teams do this, which helps you with this achievement. If they do cleanse, you will get your power, which is great, but will delay this achievement. Just play her for a couple of games and keep infecting everyone. You will run into many teams that do not cleanse at all and you should easily get 10ish of these just in that one game.

To earn this trophy, you just have to play as The Plague and successfully use your killer ability to infect survivors into the Broken status, then down them into the Dying State. VILE PURGE: This is the base form of The Plague's killer ability and is used to spread infection. You want to charge up and hit the survivor with as much of the purge stream as possible to maximize their infection faster. The more actions the survivor takes, including sprinting, will continue to increase their infection. When they are completely infected survivors will enter the Broken status and be infectious to anything they touch, including other survivors. Objects that you and fully infected survivors can infect are: Generators, lockers, pallets, Totems, vault points and the switches that open Exit Gates. A survivor that touches any of these infected objects will become infected themselves. You can also infect survivors in the Dying State or on a hook (the Hooked survivor hit box is weird, aim for their feet). To clear their infection, survivors must interact with fountains around the map that will instantly make them full health and remove the infection. The killer will get an audio alert that a fountain is being used, then the aura of that fountain will be revealed to The Plague as soon as the survivor finishes using it.
- CORRUPTED PURGE: The Plague can temporarily upgrade her killer ability to "Corrupted Purge". She does this by interacting with the corrupted fountains left behind by survivors curing their infections. When you have the Corruption, you no longer infect things. Your bile becomes a murky blood and any survivor hit with it will take damage. A single stream from Corrupted purge cant hit the same survivor multiple times the same way it stacked with Vile Purge but it can hit multiple survivors at once. When you acquire the Corruption, survivors are given a audio alert and The Plague will also give off a visual alert in the form of red sparks until she loses the Corruption. The Corruption lasts a limited time before reverting back to Vile purge. Using Corrupted Purge makes it run out faster.
