To add to the solution, you need to exit the sewer to Santa Monica and clear the parking lot.
The cheevo will pop after entering the new safe house there.

My only suggestion is the normal stuff, up to and including resetting your XBox. The reddit community is kind of small so I’m not sure you’ll get help there, maybe try the official Discord and see if there’s specific workarounds.

Bugged out for me too. Not sure what causes this but never unlocked after cutscene in safe house.

Popped for me at the safe house, just as The Red Mist quest was completed.

This has been glitched for me for about a year now.

Any solutions for this not popping, have done two playthroughs now (one solo,one coop-my host) and gotten every other achievement but this one isn't working. Didn't skip cutscenes on either run as a friend warned me about this achievement prior but no avail.

Story related. Unlocked upon completing ‘The Red Mist’ quest, after acquiring the Fury ability and before entering the confines of the horrible fleshy maze.
