There are two stages to this.
First, you will have to collect a number of items for
. Collecting them isn't too bad. You should have plenty of time as long as you don't waste a ton of time doing stuff like cleaning up achievements or goofing off. The only real hassle are the zombies, especially the gas zombies.
Second, once all the items have been collected, you will have a boss fight. You will lose all weapons, food, magazines, etc. though your health and status effects will carry over. The boss battle automatically starts once you enter the doors at the end of the stairs in Fortune City Arena. Right before you go in, drink a Pain Killer. It can be made by mixing two beers in a blender, all of which can be found next door at Americana Casino (the beers are in the Bennie Jack's Rib Shack and the blender in the center of the casino at the bar).
The pain killer will give you immunity to damage for a little bit, which is critical. I had TK's health down to about half before it wore off. There is an orange juice, spotlights, wrenches, etc. nearby and other food and weapons a little farther way.
The pain killer is essential. I failed several times without it. When I used the Pain Killer, I won the battle first try.

Man i remembered this one from xbox 360 and was not looking forward to it. I beat him on the first try with over half my health left. Wicked solution man, thank you!

I'm not hating on your guide, but it's important to note that you need to give TK Zombrex to get to the actual boss fight as noted in the other guide listed here.

Lord, Maim
Make sure that you get Zombrex for TK before facing Sullivan in order to unlock Overtime mode. Perform all of TK's errands then face him in the Arena. You will go into the final boss battle without any weapons or inventory, so be sure that you are a high enough level before starting the fight. Defeat TK to unlock the S ending and this trophy.
