Some survivors are "missable", as they are not announced.
Shopping Spree starts at 8 AM on the second day. It involves three girls but will not show up if your party is too big or there are too many survivors in available quests.
Shell Shocked starts at 1.15 PM on the third day. Also not announced, but the man is found in the American Historium found in the Americana Casino.
I missed both which took me another playthrough. (Even worse. In my first playthrough I had nearly every survivor there was, besides these 4 survivors.

Number 054 and 101 are duplicated. Number 101 is actually Stuart Holmes, found in the Americana Casino.

Filling your notebook is matter of meeting all people throughout your adventures in Fortune City. This includes psychopaths, survivors and storyline characters you meet. Most of these will come naturally by playing through the game, while other entries require some effort and timing.
Although this achievement is possible in one playthrough, it's certainly not a requirement, if you happen to miss some, you'll have a chance of completing them in subsequent playthroughs.
This achievement is best combined with "Hero of Fortune City" A walkthrough for gaining both in the same playthrough can be found here.

There are 118 entries in the notebook which are filled automatically as you meet characters in the game. These entries carry over from game to game, so if you happen to miss one you won't have to start over from scratch. Click below for a checklist that shows what area/mission to find all the characters in.
Alternately, if you have a Co-Op partner who's already gotten the trophy, have him host the game and you'll get the trophy as soon as you join his game. Your Notebook will update as well, making this trophy actually viral! Credit to Bleeds for that tip.
