So what I did in order to get this achievement was:
Go to your Multiplayer screen and select local battle.
Make sure you have two controllers. I selected Vlad the Impaler as the primary controller (I used the close and mid-range swords that you unlock after beating arcade mode with Vlad) and Hernan Cortez as P2.
Settings (after selecting the map):
7 Rounds - 150 sec. - everything 'on' (especially slice and zombie mode / zombie mode will allow the body to stand up again after its arms, head, and torso have already been sliced off).
As Vlad I just kept hitting Y - Y - Y as I stand close to Hernan Cortez. The first two hits took off the head, arm, and upper torso, while knocking the whole body to the ground. Zombie mode got the body up again and with the last 'Y' hit I took off the bodies two legs.
I don't know if it was luck, but after trying multiple other techniques these two characters and this particular button combo worked best for me.

with your technique I can get everything but one arm

Don't worry about it, I perfected my own technique which is way better.

This achievement is partially luck so don't get frustrated by it.
Use William Wallace vs. William Wallace in a local battle using his Claymore.
This is done very surgically in steps and if done right should net you the achievement as I tested it over and over again and got it MOST times.
1. Walk up and press Y to decapitate head.
2. Just barely within striking distance press X and this should sever the left arm.
3. Walk around to the side with the arm (the left) and stay barely within striking distance and press A, and this should sever his right arm and possibly one leg. It doesn't matter if the one leg gets severed or not as the steps remain the same.
4. Now there should only be a torso and leg(s) left. Hit X to cut the torso in half.
5. Now with only the leg(s) left. Hit A and it should complete the job.
The achievement should pop at the end of the round and I found this to be the MOST effective way to do it as I was able to do it with around 90%+ of the time.

how is zombie mode unlocked?

Start a local MP match and turn Slice and Zombie mode on. I did this with William Wallace press 
(left arm), go around to the other side of the Warrior and press 
(right arm and Head), press again for the torso and finally 
for the legs.
Also use a light Armored opponent like the Zulu.
