From the main menu, go to Multiplayer -> Local Skirmish -> 4 Warriors. Choose the Shaolin Monk as your first player, and it doesn't matter who else on your team, and it doesn't matter for your opponent as well. Then simply defeat your dummy controller. The achievement will unlock at the end of the match.

EDIT: Some users have reported the game freezing. It's been brought to my attention by user Wachisnoo a fix to this issue.

The game only freezes if the Monk is selected as your first character. To avoid this, pick any other character first and the Monk second, then let your first character die so that the Monk comes in second and then proceed to kill every opponent as the Monk without letting him get hit.

EDIT 2: Here's an addition to the solution from The Dren

Definitely don't skip the intros, was doing this and it locked every time, quit doing this and went right through and got the 'chevo with the Shaolin as the first character.

If you make the Monk the last character it doesn't freeze. Kill off all the other characters with your second controller and then let the Monk kill off all 4 characters that your second controller has picked.

just got it doing as CDJ 1000 said but also never skipping a warrior's introduction(don't press anything after they die). I believe the freezing has to do with the loading of the introduction.

CDJ 1000's method worked for me as of 06/18/2021. Picked Monk for last on my team and used 2nd controller to eliminate my team until him, then used monk to kill all 4 members of P2's team. Cheevo popped.

Having the monk last on your team doesn’t stop the freezing. I use second controller to kill first person on my team without skipping anything and it still freezes. I found that once someone was killed and it says perfect, the one that died press A after perfect; and so on each time. That worked.

Skirmish mode can be played locally from the multiplayer menu with 2 controllers. Each side picks four warriors to battle with and once a warrior is defeated he is replaced with the next combatant in the order that they are picked. The survivor, who must engage in battle with the next combatant, has no equipment and only minimal health replaced. This game mode ultimately ends when one player's team has been completely eliminated. For this trophy to unlock, select the Shaolin Monk as your first character and simply kill, in succession, the four idle, opposing human controlled warriors.

This trophy can also be unlocked if the Monk is not your first combatant but defeats your opponents last character without being hit.
