I found the easiest way to obtain this achievement is in Jensen's Apartment, there is a weapon safe behind the TV. This is part of the main story line very early on (during M6) and doesn't require any extra steps in order to find.
The Keypad to access it is on the Stairway leading from the kitchen into the living room. You will receive the Armor Piercing Upgrade for the 10 MM pistol along with a 10 MM pistol on the bottom row of the safe. Click combine on the armor piercing upgrade then select the pistol.
You can hack the terminal or enter the code 5375. I'm not sure if the code is the same for everyone but you can get the code from the Email's on Jensen's computer after watching the Cutscene.

You can actually get this much earlier in the game. In the manufacturing plant mission (M1) you come across some large open lab areas. The first lab is the one where you can save the hostages from the gas bomb. The second lab will have three terrorists in the main area with an additional terrorist in each of the two side rooms. In one of the side rooms, Storage Room 3 (on the right side of the room from where you first enter), there is a terrorist looking through a crate. Inside that crate is a damage upgrade. You can pick up a weapon from any of the terrorists you take down, combine with the upgrade and viola, achievement unlocked.

For those looking for the damage upgrade, it's inside a square gray wooden box marked "Fragile" in the middle of the room.
