How to unlock the Don't Reach, Kid achievement in Deus Ex: Mankind Divided - Definitive Guide

On the first floor of cellblock A (the one you start in), more or less directly beneath your cell is a locked storage room. There are a few different ways to get in here, including finding the code (which I think is inside a stuffed rabbit in the showers) or by trading for a multitool. Make sure the guards aren't nearby nor the camera pointing your way when you open the door or enter the off-limits storage area. Grab the basketball from inside here, take it to the yard, and toss it through a hoop to unlock the achievement.

Missable

The basketball is found in the Laundry room in Cell Block A. Once you have the ball, simply bring it back to the Yard and toss the ball into the hoop.

The code to the locked door is 6014, which you can find off the teddy bear in the Shower Room in Cell Block A.

It’s easier to come almost close to the hoop, jump up and release the ball at the top point (let it go, not throw it)

The basketball ball is in the laundry room on the first floor of block A, 2 hoops are in the courtyard.
