Collect all the upgrades that are located on the levels. They are marked on the map as "Item" so simply make sure you walk everywhere on the map to reveal all rooms to find them and pick them up.
Does not have to be done in one playthrough but as soon as you have picked up all individual ones, the achievement pops.
