This can be done in single player or local multiplayer. I got this in multiplayer with 2 controllers. You need a map that has the blue orb to bring you up to 200% health and the blue armor that brings you up to 200% armor. In multiplayer they do respawn after use. I think local MP would be easiest since you won't have to worry about anyone attacking you. I chose chapter "The shores of Hell" and map 9. Frag limit doesn't matter, nor does time limit. Just finish the match with 200% of each and you'll get your achievement.

I cheesed this on E2M8 with a cycle of firing a few rockets into the Cyberdemon and then quick saving until it dropped. If I took damage, I quick loaded.

I did this on the level E2M8 (last level of the Shores of Hell missions), grab the blue vest and a blue health orb to get 200% on both meters. Switch to the plasma rifle and blast the demon away while dodging it's rockets. Below is a video of how the fight goes.

The best level to do this on is the secret mission "Fortress of Mystery" in Episode 2 "The Shores of Hell" which is found in the fifth mission "Command Center". You should do this on the easiest difficulty "I'm To Young To Die" although it is not to bad on the higher difficulties if you're familiar and comfortable with the game by this point.
The entire level is short and comprised of two rooms, the first has several Barons of Hell and the second has several Cacodemons. Kill everything and then go back to the first room which has a blue armor right in the center and that will give max armor regardless of what your armor % is. The second room has a blue Supercharge orb in the center that will grant 100 health but you HAVE to be at 100 health so it will bring you to the max of 200.
Personally I did this on the third mission of the first episode "Toxin Refinery" but the secret level is far more simple and quick.

This solution worked perfectly for me. Just use lowest difficulty and you are good to go.

Did this easily on E2M8: Tower Of Babel:
-Start by opening all 4 doors around you and grab all the power-ups to get 200% health and 200% armor
-Get the rocket launcher and kill the only enemy in the level without taking any damage
-The enemy on this level is a Cyberdemon, it's a boss but it's pretty easy to kill after you get the hang of it, always do these two things: attack from a very long range and don't stay right next to a pillar as you may blow up one of your own rockets right at your face or recieve splash damage from one of his rockets! Just keep a very long distance and move constantly to a side to dodge his rockets while aiming at him and you will be ok!

Episode 1 map 3, Toxin Refinery. Play on lowest difficulty, (or hardest if you're feeling edgy) slaughter all the enemies then go to the secret area near the normal exit with the blue armor, soul sphere and partial invisibility. Cake walk to the exit.

If for any reason you are struggling with this achievement, just use two controllers and sign in as a guest on the second controller, load in E2M8 which can be found on the Shores of hell, set kill limit to 5 and 3 minutes timer to give you plenty of time. The Armor and health overcharge can be found in the centre where all the ammo and rocket launcher is, one player will spawn there to start off there right away anyway. Grab both and then just kill the other player until the kill limit has been reached and achievement should pop :)
