Early on in RoE, you are going to come to a short cutscene of a guy using the Grabber. After the cutscene, you will be able to use it. It is automatically put into your inventory. Just select it using 
or 
to cycle through your weapons. The Grabber is a bit shitty and I wouldn't recommend using it throughout your entire playthrough.
Once you have it equipped, save before you encounter any enemy that shoots projectiles at you. The best are the Imps who shoot orange fireballs and the demons who shoot green fireballs at you. Grab the fireball with
and shoot it back at them with . Do this to 20 enemies and the achievement is yours.
For other enemies, you can pick up projectiles off the ground that can be picked up by the Grabber and throw them at the enemies.
Grabbing small enemies such as Spiders and Flaming Souls and throwing them against a surface does not count towards this achievement.

Nevander
The Grabber (also known as the IPL) is a weapon very similar to the Gravity Gun from Half-Life 2. It can grab objects and toss them, and can even be used as a weapon in itself. The Grabber will be obtained right near the start of the game from a dying Marine. You will need to toss objects at 20 enemies and kill them with said tossed object.
On a related note, the Grabber is extremely useful for Forgotten Ones (fiery flying skulls) as it is an instant kill.
