How to unlock the Off with Its Head! achievement in Dragon's Dogma 2 - Definitive Guide
Guide for all 3 Medusa Achievements.
Medusa is located southwest of Bakbattahl in a small cave named "Nera'battahl Windrift".
In order to get the "Preserved Medusa" head which will give you both achievements for getting Medusas head you need to decapitate the beast before the health bar depletes two times. (If you decapitate Medusa after you've gone through two health bars you will get a Withered Medusa Head instead.)
Best way to do this is switch to the thief class who's weapon does slash damage. Also make sure you use the most basic weapon (the one they give you when you switch) because we want to be able to attack Medusa as many times as possible without depleting her health bar two times.
Leave your pawns outside or dismiss them and once the fight starts do your best to grab onto Medusa and climb to her head and attack it as many times as possible. Took me probably over 100 attacks to get the head to come off.
You will need at least one port crystal and put it in Bakbattahl the closest main city. The reason being is once you get the head immediately fast travel back to Bakbattahl and put the head into storage so it doesn't decay. Then wait 7 days in game (do a couple more to be save) and take the head back out of storage and return to face Medusa.
To use the Preserved Medusa Head open your inventory and equip it. Then open your inventory again and use the "Brandish" option and aim Medusas Head at Medusa to get the achievement.

If you don't feel like traveling back and forth with the preserved head, there is an elven shield that can also reflect her glare back at her and net you the petrifying medusa achievement.

There is a much, much easier way to do this!
-equip warfarer and get the twin blade for mystic spearhand
-level up magick archer to get sleeping arrow
-tell pawns to wait outside
-use RB to charge up sleep arrow, and put Medusa to sleep
-change over to the duo spear
-use magick cut on the head
-will INSTANTLY decapitate her head and give you a preserved Medusa head!!!!
