Select facial accessories >
Select a face guard (choice doesn’t matter) >
Tab RB to tattoos >
Select any tattoo (choice doesn’t matter) >
Press advanced >
Select any team (choice doesn’t matter) >
Advance >
Once loaded the achievement ‘Fashion Icon’ should unlock.

Not sure if this is possible if you select an existing player.
In any case, I created my own.
We look at the pictures under the spoiler where everything is located.
