Simply get to the top of dead mans peak and you will grab some kind of cranial thingy in "dead man's peak" which is the 2nd chapter of the pigsy dlc and this cheevo will pop

Ok if it still does not pop try clearing you cache files and try again, let me know if you get it or not bud.

yes, it popped! i cleared my cache yesterday and i'm pretty much sure that was the problem...

Story related and cannot be missed.

You will earn this trophy in the beginning of chapter 3: Going Down. Since there is no indication that you completed chapter 2 besides from the name of the level that you are in that shows up on the bottom left of the screen.
