Didn't unlock for me either! Had to go back and play it again up to this point and then it unlocked.

I think you need to read the note from the desk and then talk to the guy behind the desk for it to unlock.

I had no option but to take the tape from the desk and talk to the guy. If it isn’t working for you, you probably need to start a new play through unfortunately. I’m in the habit of always exiting to the menu before leaving the game on my Series X. Since doing that no achievements have been buggy.
