Rhys is an event based follower. When you are first exploring the zones the game will add an exclamation point to the map, and you'll be prompted that you discovered an event. You'll know it is Rhys event if you see a smaller than usual nest in the event location.
Make your way up to the nest and purify it, press the A button near it then mash the A button. Then you'll see a body with tentacles on the ground where the nest was. Purify that as well, after that you'll be able to talk to Rhys. When prompted allow him to join the camp to get the achievement.
The followers who join you via events are somewhat randomized. Rhys' event may take place in the Flats, but for your play through another follower event could be there instead.
