How to unlock the Make up your Mind achievement in Fallout: New Vegas - Definitive Guide

After you confront dr Mobius you will confront your brain at the top of the stairs, you will need a high speech (75, thanks rjb29) if you want your brain to come back with you. (Your brain will be back in your body once you finish Old World Blue). If you manage to convince it to return then you will unlock the achievement, not too sure if it will unlock if you let your brain stay where it is, if someone could confirm this for me thanks. (Confirmed even if your brain stays you will still unlock the achievement, thanks IndigoCerise).

Be very careful when talking to your brain. There's a conversation option that results in an infinite loop, so save before talking to it. It happens when you try to make a compromise with the brain and it remembers the interesting things you've done, like turning Helios One back online. The only way to bust out of the loop is to load a save.

This is a story-related achievement. SPOILERS

This is unlocked during the final story mission, Old World Blues. After dealing with the X-42 Giant Robo-Scorpion and talking to Dr. Mobius you have to confront your brain. There are several speech checks that will help you convince your brain to rejoin your body, but you'll receive the achievement regardless of the outcome.

There's a Medicine check of 55, and two Speech checks of 75. This is a great time to use the abundance of Mentats littering Big MT if you're just shy of 75, or a Meeting People magazine. After the conversation is over the achievement should pop.

Story spoiler free

To unlock the relative Quest: Complete the Quests named, Sonic Emitter Upgrade, Project X-13; X-2: Strange Transmissions, and have a conversation with Dr. Klein about Dr. Mobius, at the Think Tank - Dr. Klein is one of those human-brain-driven robots. You will acquire two more special equipment items by completing two specific Quests out of those three.

Following the Quest relative - Old World Blues: Activate the relative Quest, and complete that Quest simply as always, by following its Quest's logs, and the use of navigational aids. Recently acquired special equipment items are not required to be carried on your character during the relative Quest. But using that special gun is advisable, due to efficiency - also you can reset its condition upon recalibration, therefore are no Caps to invest to be required.

Xbox Achievement unlock: Upon completion of part five of relative Quest's logs.

This is a story related achievement and quest.

This requires the completion of the quest Old World Blues. A Quick run through:

  • Talk to Dr. Klein and make 'Old World Blues' your active quest.
  • Make your way to the Forbidden Zone Dome entrance, and enter it.
  • Find a solution for the X-42 giant robo-scorpion. Without spoiling anything, I say solution because there are multiple ways to finish this part of the quest. Have fun choosing.
  • Find Mobius' Lair and deal with him. Again there are multiple way to conclude your business with Mobius. 
  • Get your brain back... or deal with it. Again with the personal choices. 
  • And reason or destroy the Think Tank

Note: The final step of this also completes the following achievement: "Outsmarted."

This trophy is obtainable during the Old World Blues quest once you reach the Forbidden Zone. Be prepared, as once you arrive in this location you have to face a mini-boss enemy where you can disable (if you have the skills) or defeat it. After dealing with the situation, you’ll make your way into the lab and will need to talk to Doctor Mobeus in-which you’ll learn in the conversation that it will be up to you to convince your brain to want to return to your possession.

Go up the stairs where you will be able to locate and talk to your brain. It is advised you make a manual save before you talk to your brain. There is a possible bug where if you happen to have completed Come Fly With Me and That Lucky Old Sun from the New Vegas campaign in your quest journal, and you choose one of the compromise options, you could get caught where your brain says "Jason Bright and his followers launching into the vast unknown" and "HELIOS one coming back on-line" in an infinite loop. Your only way out of this would be to reload your game.

The easiest path to this trophy is having a Speech Skill of 75 and a Medicine Skill of 55 which will turn the conversation into your favor rather easily. For the specific dialog choices to choose you can refer to the video:

After completing the quest "X-13 Attack Of The Infiltrator" , we will be given the quest "Old World Blues".

We go to “The Think Tank” , where we talk with Dr. Klein (the same one who gave us all the quests). The marker on the map will update, inviting us to visit the Forbidden Zone Dome Entrance , which is located in the northern part of the Big Empty . But before you go there, I recommend that you make sure that you have enough ammo, stimulants and radiant, because otherwise the chances of survival will be very low. When you reach the location marked with a marker, go inside. If you are equipped with stealth armor, then she will immediately say something like “This look is pretty tough.” And this is true, because for the first time since part 3 we will have a battle with a boss, which will be the Giant Roboscorpion.
When Roboscorpion is finished, go to Dr. Mobius and talk to him and you can kill him if you wish. One way or another, after the conversation, return to the “The Brain Tank” capsule (it is located right there) and talk to your brain, convincing it to return back to your head. If you convince, you will receive the achievement for “Make up Your Mind” . Before talking to the brain, I strongly recommend saving.
