In order to unlock this achievement you will need to complete the following steps.
1. Find the secret level portal, which is hidden behind a wall in (I think) a random level. You can break the wall placing an explosive with LB. The secret level portal is usually found in the first stages. I think it cannot be found past level 2-3 but I might be wrong.
2. In the secret level, find the mini-boss (a hooded-mage kind of character surrounded by books) and kill him. He will drop a book, that you have to pick up. Unfortunately the secret area has no mini-map but shouldn't be too difficult to navigate through.
3. Exit the secret level and reach level 4-3. In this level, find the library and fill the only free space there is on the shelves. Achievement will unlock here.
ADDITIONAL INFOS
If I recall correctly level 4-3 can only be reached if you play normal or hard mode, because easy mode ends sooner.
Also be aware that you have to pick up the book and return it in the same run. If you die after collecting the book, you will have to find the secret level and kill the mini-boss again.
In order to find the secret level portal I suggest buying the upgrade that will help you find breakable walls by showing cracks on it. During your run you might also find a scroll that removes fog of war from the map, thus showing every room in the level including the hidden ones. But no one can assure that you are not already past the secret level entrance.
