This still works, but the website has changed. You'll need to visit
http://www.ea.com/uk/football/fifa/game-face
and log in using your EA account. You'll be asked to install a plugin (Unity) unless you already have it. On the website, either create your own Game Face or just use one of the premades. Be sure to hit "Save" and actually check it goes through - sometimes it returns with Error.
In game, you'll need to log in with your EA account (which usually is already attached to your gamertag). Go to Virtual Pro, create one, then download your Gamer Face for the achievement.

Looks like this no longer works - I presume there is now no way to get this achievement since the servers have been turned off.

First of all your going to have to go to this link.
Download the software needed for that by following the insturctions the website gives you. Apparently, this is not available on a Apple Mac, so if you one of those I'm sorry, still try it though just in case.
***Once that is complete you're going to have to go onto Fifa 10, go to Virtual Pro >> Download Game Face, and it should pop up after 100%.***
