There are twelve Stellazzio coins, plus one hidden coin, to be found in different areas at any point during the game.
This achievement is not missable as the items can be gathered any time.
Take these coins to Treno and offer them to Queen Stella. You will receive a reward for each coin you hand over to her. The prize improves with each coin you hand over, regardless of the order in which you do so.
THE 13TH COIN
After you give Queen Stella all twelve Stellazzio coins, speak to her again. This time, she senses that something may be missing. Upon agreeing that something is missing, the Queen charges you with finding the hidden thirteenth Stellazzio coin. To aid you in your quest, she returns all twelve Stellazzio to you, so that you can read the clues inscribed on the backs of each one.
The clues from the twelve Stellazzio are as follows:
"...headed east."
"...Through the forest and over the mountain..."
"...by the river..."
"...the cape...the sun was setting into the ocean..."
"...waiting for the sun to set in the ocean..."
"...watching the sunset from the cape..."
"...opposite the direction of the sun..."
"...always looked at his shadow, until one day, he decided to look away. He walked up a hill..."
"...dashed through the night, defying the chilly northern wind that was stinging his right cheek..."
"...ran up a hill toward the sun and fell asleep..."
The clues on the last two Stellazzio coins, Aquarius and Pisces, serve only to wrap up the story.
Assembling the clues, the player can determine that the thirteenth Stellazzio is up a hill above a river, beyond darkness where the sun seems to be setting, somewhere to the northeast.
Thus, if the player leaves Treno, and head northeast to Quan's Dwelling, they'll find the Ophiuchus in the spot where they previously found the Scorpio Stellazzio.
Incidentally, the back of the thirteenth Stellazzio provides the conclusion to the story inscribed on the backs of the other twelve Stellazzio.
Head back to Treno and return the twelve Stellazzio to Queen Stella, plus the Ophiuchus. In return, you receive a Hammer, which can only be equipped by Cinna.
THE HAMMER
This item can be used to synthesise the Tin Armour from Hades in Memoria.
If you finish the game with this item in your inventory, there will be an extra scene added to the ending. Better yet, when Cinna comes out in the ending, he will have a hammer in his hand. Great.

Each Stellazzio is named after a sign from the Zodiac. None of the coins are missable and you can go back and collect any of them once you have access to the Invincible in Disc 4. Most coins are found on the ground so look for the ! field icon when in the right location to find them.
The locations of each coin are listed below, in the order that you can find them:
Aries - In the ground floor of Dali's windmill, in the back of the room behind the crankshaft (Disc 1).
Cancer - Behind an overturned cart on the left of the second screen of Burmecia (Disc 1).
Scorpio - In Quan's Dwelling, slide down the large rope towards the back of the room and search towards the bottom of the screen down by the springs (Disc 2).
Gemini - In Treno's entrance, throw 10 Gil into the fountain on the left hand side of the screen 13 consecutive times (Disc 2)
Taurus - In Treno’s slum district, behind the item shop’s stand on the back right corner (Disc 2).
Virgo - In the Black Mage Village’s inn, at the bottom of the ladder next to the bunk beds. In between the ladder and the decorative chest.(Disc 2).
Libra - Behind the fountain in Madain Sari. Just behind the broken portion of the fountain(Disc 2).
Leo - In the back right corner of the room near the Neptune Statue (the one with the glowing red trident) in Alexandria Castle (Disc 3).
Sagittarius - On the left edge of the screen at the bottom of the grey machine with a pipe coming out of it. Left of the Gysahl Pickle cart in Lindblum's Business District (Disc 3).
Aquarius - Behind the middle pillar on the right in the entrance to Ipsen's Castle (Disc 3).
Capricorn - In Daguerreo's entrance, at the dead end of the canal on the right (Disc 3).
Pisces - Aboard the Invincible, in the treasure chest left of the warp pad (Disc 4).
Ophiuchus - Return to Queen Stella after collecting all 12 coins above. She will return them to you and ask you to find the 13th and final coin, using the clues on each coin. Return to Quan's Dwelling and the exact spot where you found Scorpio and you will now find the Ophiuchus coin (Disc 4).
Returning each coin will result in a reward from Queen Stella. After 10 coins, you will receive the Earning the Queen's Favour trophy.
After getting all 13 Stellazio the Queen will reward you with the unique item the Hammer which can be used as a component to synthesize the Tin Armor (Steiner's best armor) at Hades synthesid shop in Memoria.

The Hammer is a unique item that can only be obtained by returning all 13 Stellazzio coins to Queen Stella in the North of Treno. You don't need to speak to Queen Stella to start this quest and can collect a number of the coins before you first go to Treno in Disc 2.

The, Pants, Party
The Hammer is a reward you will receive from Queen Stella after collecting 13 unique coins throughout the game and giving them to her in Treno. None of the coins are missable, though thankfully they ARE included in the missable walkthrough I linked in the Road Map. Following that, you will get them all along the way. Their locations can also be found here with a video to help if needed.
After collecting 12 coins, visit Stella and the last one will become available. Go grab it and return once more to receive the Hammer and unlock the achievement.

After you find the 12th coin, go to the queen in Treno and give her all the coins. It turns out that there is a 13th coin. Go back to where you got the Scorpio coin. At the same place you will find a Scorpio coin. Bring her coins and she will give you that desired Hammer))
