You must start and finish the entire game with Haggar. Doesn't matter the difficulty or how many times you continue. Just make sure you start the game with him and do not "join a game" already in progress even on level 1 or you will not get the achievement.
Also be careful when you press continue not to accidently pick another character or that will also void this achievement.
This can be done in a solo, local co-op, or XBL co-op game.

Happend to me by accident (the analog stick seems a bit too sensitive in the menu's). You can still 'restart' (which will take you to the most recent autosave point) and continue the game with the character you started to still get the achievement.

Join a game on fisrt level, finish the game -> no achievement.
Restart the game for the begining, finsh the fisrt level -> achievement.
It's seems that you have to complete all levels with the char, no matter the order.
