Just thought I'd add a bit of clarity as to how to unlock this achievement.
In order to unlock this achievement, first press
to access your Heroes, and then select one, then select 'Customize', then press 
.
To change your color swatch, you just have to select 'Colors' and then choose one you're not currently using. To change your hero's Symbol and Paint Pattern, you must first select a body part (for example, the right shoulder), then change the Symbol and Paint Pattern under their respective tabs. Once you've changed all three, the achievement should unlock.

Fairly easy achievement. On the Hero page, select your desired hero and then select Customize. From that new menu, hit to move over to the Appearance Tab and then change your heroes Symbol, Paint Pattern and Color Swatch to something new.
Once you back out and save the changes, the achievement should pop.

But Xbox button icons won't load in this site (Back, LB). When you're free, edit out those and change "achievement" to "trophy"

Pretty easy and fast trophy to get. From the Main Menu, select Customize then Heroes then Appearance. Here you can select any Paint Pattern you wish, then, if you move the tab with 
you can select the Symbol tab and pick another choice. Go back and select Colors. After selecting any color swatch you desire, the trophy should pop right away.
