The trophy is won at the last location. We clear the location, leave 3 or more doctors, lower our HP below 10% and quickly move around the location and, with the help of doctors, restore our HP to 90+%

The trophy can be taken anywhere in the game. I would recommend taking it from the very beginning, let the mob lower your HP below 10%, and then just kill everyone around, getting spheres and restoring health. As for the final level, this is extremely inconvenient because one portal restores a maximum of 45 health points to you, in total you can leave 3 of them at the level. After killing the bosses, you need to take the portals immediately. If you have more than 200 health points, you will not be able to recover.
