You can do any area in any chapter on any difficulty in Co-Op to get this achievement. In fact, you can even do the very first level called Bootcamp in the Prologue Chapter and once you complete the area, you will not only get this achievement but also the Iron Man achievement (complete the tutorial level).

I started the game a few days ago with a friend through Xbox LIVE campain, but at the end of the first level, I was the only one to get this achievement and the Iron Man one. Does only the host of an online game get the achivement? Because we are at the beginning of chapter 4, and no achievement unlocked for him but did for me, the host, with the two related achs I had.
This way, we're not sure he'll get the achievement for finishing the game on hard (we started directly in hard).

This achievement can be done alone with a second controller. 1. Campaign
2. Local Co-op
3. Prologue
Just Move the second controller a little bit at a time as you make your way through the Boot Camp and after that when the enimies start attacking clear out enimies and and complete the area for the achievement to pop.
