You just have to get 25 CoOp-Assists in one level. I got this one in the first Wavering Hills Asylum level. It's not required to play a level with someone else, I got when I played alone with computer controlled helpers.

This can be done in ethier single or co-op play. Easiest way to do this is to throw out your grenade (LT) into a bunch of ghosts & let your team mates get the kill.
If done correctly there will be a notification in the top middle of the screen with "team assist"
Alternativly this can also be done when your team mates throw their grenades, just shoot & kill any that enter the area of effect for the team assist.
This needs to been done in one level & can be unlocked with an unleveled character.
