It's very easy to get this achievement in the first playthrough. The only thing to do is to reload save after saving. And by the way there's 94 missions in the game and you have 30 hours to complete them so you do three or four in one hour and all will be fine. At a moment I had only 30 missions done at 15 hours of the game, then I really started the quests and I finished the game within 25 hours without skipping any cutscenes (exept after failing the mission) or taking any taxi.

People talk about loading up saves and skipping cutscenes. Would make sense if the achievement was for 15 or 20 hours. But 30 hours is a lot of time. Watch cutscenes, heck even do side missions; The key is to use taxis wherever you can. I got a couple of side missions done, still managed to get the achievement. Don't let getting this achievement ruin your GTA 4 experience.
This is easy, just don't stop doing missions do one go to the next one.

This may very well be one of the easiest achievements in the game. Just play all of the Story Mission first, do NOT play any side missions like the car deliveries and things like that. You should easily be able to get this achievement then. I got it with about 23 hours of playing time.

I suggest useing taxis to get from mission to mission. If you fail a mission just load up the auto save. Use cheats to make it go faster if you have one man army, cleaned the mean streets and walk free. Which you should do on your first file. The cheeve will pop after the credits just so you know.

Yes, I can confirm that it is possible to get this achievement with a casual play-through of the story missions only. I, unfortunately, ran through them not knowing how much time would ultimately be needed and so completed the story in a little over 13 hours. I could have enjoyed it more by watching the cut scenes and driving to locations more often.
What would have been helpful to convince me to play casually was a milestone listing of times. So taking my rushed play-through and adding a proportionate amount of time to complete in 25 hours, I provide this time guide:

Remember to save often as some missions can prove particularly troublesome requiring multiple attempts. For me that was Pest Control and the final mission.
The final missions require time to pass between them before they will appear. Sometimes telephoning the character will speed things up a bit. Sleeping will move ahead 6 hours of in-game time. For one mission I remember 30 minutes passing. I guess you are suppose to do the side missions while waiting for these missions to show up. If you are good for time, go ahead.
Please note that the completion percentage provided above is for playing the story missions only bar for one jump which occurred during a boat mission.

I got the achievement fairly naturally, but my brother took 40 hrs.
Dissapointed, he reloaded a save that had 18 hrs on it. putting in a modest effort of not screwing around, he beat the game in 24 hrs.

Here's what to follow if you want to quickly complete the main storyline:
1) Only do missions that appear on your map. e.g if it says R for Roman or B for Brucie, you only need to focus on those.
2) Quick way to fast travel to missions is by taking a taxi and pressing A to skip (downfall it changes the time of day depending on the distance.)
3) Ignore going on dates with people unless it's already on the map and it's part of the mission (don't bother taking the initiative to call people.)
4) Don't do any other side achievements. e.g cab driving, special delivery and so on

You can go through without skipping the cutscenes. I completed this in 23 hours, replaying 7 tasks.
But still, don’t forget to buy guns and go to your heart’s content. Have a good game.

Also, after mission failures, you can go to the dashboard, and then back to the game, the time spent since the first load will not be added (it is saved in statistics when you save the game). This is how I replayed the last mission.
