I'm not 100% sure about this, but this triggered for me during the "We Go Together" song in the Rockin' Rydell mini game. Right near the end of the song a different instrument to a guitar comes in when you play the notes (possibly a violin). I hit most of this sequence (though I don't think it was 100% perfect) and the achievement popped. Most of the rest of the notes in the song were hit ok but they were certainly not 100% perfect. It's worth a go to see if it works for you.

It popped for me as well and I hit 50% of the notes at most. Never saw a chipmunk. This Rockin Rydell mini game is so broken...

I didn't get close to hitting 50 and I still got. Only thing I noticed was the voices started sounding like Alvin and the chipmunks at end of song

From the Main Menu, select Extras, Minigames and select Rockin’ Rydell. Here, you need to select the song We Go Together.
The objective is to press correctly the very last sequence of the song with no errors. You will notice this part when you hear a chipmunk singing at the end, so do your best and hit the right buttons as they show in the screen to unlock this trophy.
If you complete the sequence and the trophy doesn’t pop, try again. Maybe you didn't hit enough buttons correctly.
