To achieve this ending you have to elect Duncas as the high king, and then reach the ending without an alliance amongst the factions. This is 1 of the 5 endings and to unlock this, as with all others, you will need to make certain choices throughout the game.
For this ending you need to make sure that you DO NOT have all of the factions set to 'Friendly', you can have some of them 'Friendly', just not all of them, so that they do not unite during 'Together for Teer Fradee'. You also need to choose Dunncas as the High King during the quest 'A New High King', then of course DO NOT side with Constantin during the final cutscene at the end of the game.
The achievement will unlock during the credits.
