Everyone has to enter a hot streak (50+ note streak) at the same time. Can be on any difficulty. Best done while attempting "'Metal Miliia'" achievement

Not every song has 50 phrases on beginner vocals.
Easy songs to get this are 'The Unforgiven' and 'Nothing Else Matters'.

This can be be done online with your instrument and a pad for vocals.
Have your own instrument on whichever difficulty and have the vocals on beginner and use the fan trick. From here go online and join a band quickplay game and hope you get the rest of the band, you should only need a drummer + guitarist or two guitarists (depending). Simply play through and wait till your vocals has amassed a streak and make sure everyone get the streak along with the vocals.
It is worth noting that not all of the songs (about 60-70% I'd say) have even 50 phrase sections for the vocals, so cannot amass the needed amount. One that i know does however is Mercyful Fate which has around 89 sections, more than enough.

What do you mean with "fan trick"?

The fan trick is where you put the singer on beginner difficulty so all that is needed to hit the notes is a constant source of noise. Putting the mic near a fan (or even loud tv etc.) will make the singer get 100% without you needing to touch it.

Make sure that you're in a 4 player band (locally or online) and just keep playing while missing little amounts of notes. You'll know you got the trophy when all four highways start glowing yellow and the message that says "50 Band Note Streak" appears.

A hot streak is when you hit 50 notes in a row. Once all the members of your band do so, you will have this trophy. If you are having trouble with this trophy play on easy or beginner on the song "Stone cold crazy".
