i think the 49% of people who dont have this achievement were too distracted by the pic for this achievement.

When you're in career mode, you should have an option to make a tattoo either arms or torso. Just select that option, make a tattoo, name it, and then save

Go to Career and pick any instrument. From the career menu pick Edit Rocket, go to Body and select Tattoos. Pick edit and new layer and add any image on your character. Then go back and select Save and New Save. Name it anything and accept. After it saves, you'll get the trophy.
