To get an 11x Band multiplier you must all activate your Star Power whilst keeping your combo at 4x(will go to 8x in SP) when you have set a Band moment off. The Band moment is triggered when you all hit the orange, fiery notes you see at certain points in songs.
As it says, you can do it with any 4 player/instrument combination so do what you are most comfortable with. There is no difficulty requirement for the achievement either so don't feel like you have to be on Expert.

I did it like Hitoshura did, it's worth noting that wireless headsets can be connected and used as Xbox 360 headsets, and you want to turn Star Power Tap off in each controller's options menu so that you can manually activate all star powers when you get a band moment.

My group had a few issues getting this achievement as people were activating SP at the incorrect times.
We found it best to activate SP as soon as everyone saw the first flaming note (before the band multiplier kicks in). That way everyone already had SP activated (and a maximum multiplier) when we were awarded with the band multiplier.

I used the fan trick on an imported GHWT song "no sleep to brooklyn". 4 headsets set up on a fan all set to expert, then press A on all pads on them all at the same time. I also got the "Apostates of Orthodoxy" Achievement at the same time.

One easy thing to do if you have two guitars and two mics and no friends is put the difficulty on Beginner for all instruments. Have both guitars be bass guitar and put the two mics in front of the fan. The mics will be fine, and since the guitars are both bass, they'll have the same chart so you can just strum both of them at the same time. Do Bleed it Out by Linkin park and just strum both guitars. Near the end of the song, there's a band moment right before the bass gets a break, giving you time to hit star power on the mics and the guitars without worrying about messing up and the achievement should pop.
