You can change difficulties using left or right before selecting a song.
To unlock the Extreme difficulty, you must first 'Clear' a song on Hard to unlock the difficulty for that song. This requires you to get enough points to pass the song, simply getting to the end will not be enough. Personally, I found the easier of the two free songs to be Out of Gravity (the second song) as the rhythm is more basic and easier to keep up with.
