After all the updates previous method and many others don’t work.
But After many tries I finally found one.
On your ship you have to push “square” to access free warbond “HELLDIVERS MOBILIZE”. You’ll need to buy your way to page 5 and buy the “DP-40 HERO OF THE FEDERATION” body armor, helmet and cape. Not sure if helmet and cape are necessary, because armor is the only piece with additional stats.
You can wear it in the armoury or right before the mission. Trophy doesn’t work with other armor I tried.
Next choose any mission and call the Supply pod (DDUR). Stay in the mark until it will land on your head. As soon as it’ll break your limbs and will kill you, you’ll get the trophy.
It can take several tries to break all four limbs.

If you have the armor that gives you 50 % chance to survive lethal damage you can walk into mines.

The way I got this achievement was by my teammate throwing a G-16 Impact Grenade at me whilst I was wearing the DP-40 Hero of the Federation set, which gives a ‘50% chance to not die when taking lethal damage’ armour passive. Hope this helps anyone who’s struggling to get this achievement!

Easiest way to do this is with a friend. Have him shoot you in each leg and arm with the starting pistol (P-2 Peacemaker). You need to have an Armor Rating of at least 100, so equip a Medium or Heavy armor. Light armor will not work as you will die in 4 shots.
