When you win a war you can claim territories. When that territory has a city, that city will become yours.
However, since that city belonged to another empire at first they might have other civics in place than you have.
Therefore, these cuties will start to ask you to change your civic choose. There are only two options if this happens, accept our refuse. Refuse all of these requests and once you win the game, this cheevo should pop.
Since you need to win the game, my advice is to set up the game as easy as you can. Preferably with just one basic AI.
