It's worth mentioning that some medals may disappear between playthroughs, therefore it's recommended to try to obtain as many during one playthrough, followed by a run through the challenge room.

Just to be clear, this guide does cover everything post-patch. However, as of 08/11/15 some of the links to the pictures in the guide appear to be broken (this doesn't affect this achievement directly, but it will affect the Private Eye achievement). A few tips for medals that gave me problems:

Flood: Flood the corridors after leaving the loading bays.


Gravity - Launch 2 Malthusians over the side of the shuttle plaza

No matter what I did, I could not get the Deep Breath medal to unlock while playing on X1. I loaded up the game on my 360 and got the medal 1st try.

Play this on the 360, Xbox one is oddly not the same. And heads up, don't fail a hack, you will need to restart the WHOLE process over.

Deep Breath ruined the entire game for me. I can't be bothered to start over or even continue the game now.

I lost two Medals. Flood and Geek. I selected Act 1 and went to the end of the game again without failing the hacks and it worked for me.

You don't need to restart the whole game to get Deep Breath, just chapter 2, It worked for me.

I can't get Humanitarian to pop at all. I did a complete playthrough on XSX, started again from Act 2 on XSX, did a new playthrough on X360 (patched version), did another playthrough on X360 (patch removed), and nothing I'm doing is working.

Edit: instead of shocking an enemy with the damaged wires, I had to use Energy Rounds followed by Semi-Auto Rounds. I did it after unlocking the Super Sonic medal.

Not a great guide in my opinion! Better than nothing but very easy to miss collectables!

But be wary of a missed document collectable on a wall at around 1h 7 min that he goes back and show at 1h 10 min. However if you do what he did then you can´t really go back if you go into the vent so be sure to pick one more than he did before going in to the vent in this place. I recommend continuing watching before doing anything from 1h 6 min mark to 1h 11 min mark just to be sure. You will suddenly see him back in the place he were before going in to the vent.

Also be sure to always disengage when hacking if you are about to run out of time. Also never change to another ammo than supersonic rounds except when going for other medals but if you do then be sure to reload last save when you get a medal using other ammo/rounds.

If you have accidentally screwed up the Deep Breath medal by scanning the wall and not getting the medal, there's a way to get the medal without starting over completely from scratch.

There are individual save files for each act. So I got my character to Act 3 with 46/47 medals. Then I moved all my save files (including options) to my cloud storage and deleted them off my hard drive. Then I started a brand new game on my Hard Drive and did a speed run to Act 2 on Easy. Once in Act 2 I got the Deep Breath medal and then proceeded far enough in the level to get a check point.

After doing this, I quit out from the game and went back to the storage options on the Dashboard. I then moved my Act 3 save from my Cloud storage back to my Hard Drive. I reloaded the game, continued the game (in Act 2) to make the game think I had the Deep Breath medal. Then I exited the game (to the game menu, not the dashboard) and started a new game from Act 3. The game then registered I had 47/47 medals but didn't give me the achievement. I proceeded into the level, killed the first enemy I found, looted his corpse, and I got the achievement.

if a miss some in one playthrough the rest of them stay at second? Or i must do all one more time?

The medals should persist through multiple play throughs. It's just the medal described above can't be gotten again if you scan the wall. The instructions above will tell you how to get the last medal if you have 46/47.

I'm a bit confused. On my first play through I got the documents, but didn't scan the wall. I then went through the game again scanned the wall and got both documents in one breath and the medal won't pop. Any ideas?

Yeah, that medal is glitched. That's why you have to go through all of the steps outlined above.

One thing more to mention in addition to the xbox360achievement guide that gave me a free card to another playthrough.

You have to successfully hack ALL devices you come across and have to play through the game with ONLY using your Sonic Rounds. They are lethal after the patch, so there is not much problem with this one. The guide tells you this at the end ...

The by Caduceus mentioned Deep Breath Medal in the beginning of act2 gave me troubles too. But its really easy if you know how to do it. At the beginning, turn around inside the elevator - dive down, take a right, collect both documents from the corpse (2 times X), turn left, scan the wall and speed back.

Another troublesome one was the Flood Medal in the beginning of Act3. To achieve this on - open the first door with the Camera (it stays open this way), destroy the 2 glass walls in this corridor, in the next room open the door you came from with the camea to flood this one. Dive to the opposite door and in the next corridor destroy the glass to your right and the 2 in the right corridor - NOT the one left.

For some medals where you need to kill someone (Stealth Medal in Act 2 where you need the gel rounds and Humanitarian - just reload checkpoint after achieving the medal - it will not disapear and you can kill the guys with Sonic amunition).
If you see you will fail at a hack, just hit B and try again, it will not count as a failure.

Hope this made some points clearer and spares you a second playthrough.

not all stay ... some do stay and some disappear on a second playthrough. But cant tell you which are those ones, to long ago since I played this one ;)

So if I "Load Game" from Act 1 will all my medals still be saved from the last play through or do I have to do them all over again?

load game and restart wil be handled as one and the same. Some Medals stay and some disappear and you must unlock them again but with reload only in the specific act).
If you messed up one long time medal like "no kills" or "no hacks failed" in a specific act, replaying will not unlock your medal. I remember cause I tried ;)

As a side note, if you are having trouble getting the Deep Breath medal, (as the way the guide stipulates confused me at first), there are some steps to take. If you consistently repeat the requirements to get the medal and it does not appear, go to the Dashboard. From there delete your saves, including the save that says "Options." This will delete your medals, documents and objects. Reload your game and speed-run to the point in the elevator shaft.

Go to the top of the shaft and then swim down. Grab the two docs AND scan the Mavi symbol which will now be able to be scanned. As long as you do this and resurface in a breath, the medal should appear.

For performing any unusual actions in the game, you will receive medals, collect all the medals. (medals received can be viewed in the statistics menu)

When receiving some medals (for example, In Water), glitches were noticed, so you should patch the game by downloading an update (i.e. you should not play offline).
