I am missing the first 2 in Act 2, there should be a dead body directly in front of me but there isn't. I even played a new game and it still isn't showing up.

I'm starting to think some of these locations randomize. I just watched the videos in the other solution trying to find page 21 in Act 3 and it clearly shows document and object locations that I didn't have during my first playthrough.

I'm noticing the same thing. There might have been a patch or something, because a lot of the green bodies and a few documents moved locations from the videos.

Just to be clear, this guide does cover everything post-patch. However, as of 08/11/15 some of the links to the pictures in the guide appear to be broken. Having said that I was still able to find the objects/documents where that occurred by following the text description and doing a little investigating.

Object 13 also not counting for me.

Edit: Continued forward anyway, achievement still unlocked. If Object 13 doesn't count for you, don't panic and keep going!

Great guide but watch out for glitches, I had a document that I collected but didn't register. I suggest if you are doing a play-through check your document count after every one you collect.

I had to delete all my save files and start again to be able to collect it again.

does it have to be in 1 playthrough or can i get them by replaying the acts ?

I failed my first hack. Can I just replay act 1 or do I have replay the whole game to get the medal?

How do you check what ones are missing because appapently i have 53/55

Not a great guide in my opinion! Better than nothing but very easy to miss collectables!

But be wary of a missed document collectable on a wall at around 1h 7 min that he goes back and show at 1h 10 min. However if you do what he did then you can´t really go back if you go into the vent so be sure to pick one more than he did before going in to the vent in this place. I recommend continuing watching before doing anything from 1h 6 min mark to 1h 11 min mark just to be sure. You will suddenly see him back in the place he were before going in to the vent.

Also be sure to always disengage when hacking if you are about to run out of time. Also never change to another ammo than supersonic rounds except when going for other medals but if you do then be sure to reload last save when you get a medal using other ammo/rounds.
