"First go back and play LEVEL 42 on the Ancient World Campaign
Make sure you collect 2 Gems, DO NOT COLLECT 3!!!
Finish LEVEL 42
When you move on to LEVEL 43 you must collect at least 1 Gem
This will give you extra life #1
There are 2 more extra life power ups scattered on this level
If you have to knock out every brick to find them and skip the Warp do so. I played this level 3 times and every time there was 2 extra lives."
Just adding that If you die until you get the two extra more lifes, the trophie will count.
