This Trophy is story related and will be available during your Evil Karma play-through.

The Trophy pops at the end of the "The Price" mission, Towards the end of this mission you will have make the choice to save either six doctors or Trish. You must selfishly save Trish, sacrificing group of doctors for this Trophy to pop.
