You will get this after completing the last mission in the game ( Mission 8 )
There is a trick to this: Play on Easy until you get to the last checkpoint and then quit out to Headquarters. From Here you press start, change the difficulty to Formidable and press Start again to save your choice.
Return to the opening screen, choose to continue game and use last checkpoint.
This will take you to the last part of the game, where you just have to shoot Ultimo in the head ( with rockets / missiles ) and then its over.
Cheevo will pop after the cut-scenes. No need to battle this out on any other difficulties because it is extremely difficult, with all the stupid camera angles and flight controls.
