Whenever you're in battle there will be an invisible wall surrounding the area.
If you run into these walls continuously a yellow meter will appear with "ESCAPE" written above it.
If you fill this meter you'll escape so just don't run into the walls and you'll be fine. Keep in mind, though, that some enemies will have attacks that can knock you backwards into the walls and that fills the meter too even if just a tiny bit.

For this trophy you must complete the game without escaping from a single battle, to escape you must run against the edge of the area during a battle and fill the escape part. Quite frankly there is no real reason to escape from battles anyway, there're too many benefits from it when progressing in the main story so it would be weird to escape at all.
If you don't want to fight enemies just avoid them in each room, if you so happen to get caught just get the fight over with. When you accomplish this the trophy will unlock at the end of the game with the difficulty trophy.
