This can be done during the quest "Leaky Faucets" in the Teeth of Naros part of the game.
You'll need to have a reasonably high level of "Detect Hidden" for this as well.
After you've entered the undersewers and are working towards the three levers to clear the cisterns, just before you reach the first there is a hidden door behind you... follow the path through this and kill a couple of Pteyr's in an area with a large oblong shaped hole in the floor... if you check your map there should be a white arrow on it denoting an NPC described as a "Strange Man".
Jump down the hole and speak to him... achievement should pop during the conversation.

I found him while grabbing the Circlet from the tomb.
I didn't leave right away & followed the paths, ladders & jump spots to the top right of the map after the first hidden door.
After killing some rats & pteyr's there is a jump spot into some water. He is there.

You will need to go into the Undersewers for the main quest "Shapen in Iniquity." You'll be going here to retrieve an item from a coffin. Once you've done this, instead of going back out the way you came, continue down the path through the Undersewers. You'll eventually be able to jump down a hole to where this Almain is living. He's really the only NPC in the area, so you can track him on your map by searching for the white arrow.
