How to unlock the Reborn achievement in Kingdoms of Amalur: Re-Reckoning - Definitive Guide
This achievement is story related and unmissable. It unlocks when you complete the prologue. The prologue, itself, is well scripted and easy to follow. Nevertheless, there are some important things to bear in mind.
First, you must select your difficulty. There is an achievement for finishing the game on hard or very hard difficulty. You have the option of either one or two playthroughs. One option would be to play the game on casual (easy) or normal to get most of the achievements and a speed-run on hard or very hard for the difficulty related achievement. I recommend the other option to play on hard or very hard. The game is not challenging in its combat and you have the overpowered reckoning mode to help you out in the more challenging fights.
Second, you must select your race and patron. I recommend Dokkalfar: It is the only race with a combat bonus—stealth +2. It also provides persuasion +1 which helps for a rather involved persuasion-related achievement.
Third, during the prologue itself, there is an opportunity for a persuasion check. It is right after the tutorial section with the spiders and the staff. When you enter the next area, before you speak to the NPC (Encel), make an insurance save. Encel gives you some tips and says farewell. You have a conversation choice in bright green, “I need more help." This is a persuasion attempt. You need to succeed in 50 of them for an achievement. There are about a hundred opportunities in the game so this one is not a must-have. However, it is a very easy one and you might as well take advantage of this low-hanging fruit to get a good start towards the achievement.
The prologue ends with a more challenging fight than the ones that preceded it—a Rock Troll accompanied by two soldiers. Dodge-roll away from the troll () and defeat the two soldiers first with the weapon of your choice. The Rock Troll has powerful attacks but telegraphs clearly.
When you reduce the Rock Troll’s large health bar (shown in the upper right), you will get a button prompt. Press the indicated button rapidly. This increases experience gained depending on the percentage indicated. The percentage increases proportionate to how fast you pressed the button prompt. The button prompt is random and could be either , , , or .
After the fight, exit the dungeon. This completes the prologue and unlocks the achievement.

Complete the tutorial portion of the game and it should pop when you leave the tower.
Complete the tutorial level.
The basic thing to remember is to use to evade often if you're going for the Hard mode trophy.
The troll towards the end is the "hardest" part. Although the entire level is easy as even on hard, as long as you avoid damage everything goes down pretty easily. Also, if you do muck up, the game is generous with health potions early on, so take advantage of that.
Once you've got out of the well abd see daylight for the first time, the trophy pops.
